Qigong Services Agreement & Contract

Parties to This Agreement

This Qigong Services Agreement ("Agreement") is entered into between Moving Qigong LLC dba House of Guanyin, represented by Instructor Katrina T. Olurinde (Coach Katrina), hereinafter referred to as the "Instructor," and the individual identified below, hereinafter referred to as the "Client."

Services Offered

House of Guanyin offers private one-on-one Qigong instruction sessions. All sessions are approximately 60 minutes in duration. The Client agrees to select one of the following service packages below. All invoices will be sent electronically via email.

🔀 Custom Mix of Virtual & In-Person Sessions Client requires a combination of virtual and in-person sessions. A specialized invoice will be prepared and sent electronically via email based on the agreed schedule. Virtual sessions: $25.00/session · In-Person sessions: $45.00/session.

Payment Terms

All invoices are issued electronically via email at the beginning of each billing cycle. Payment is due within 5 business days of invoice receipt. Accepted payment methods will be communicated by the Instructor. Late payments may result in a temporary suspension of sessions until the balance is resolved.

Cancellation & Rescheduling Policy

The Client must provide a minimum of 24 hours advance notice to cancel or reschedule a session. Sessions cancelled with less than 24 hours notice will be charged at the full session rate. No-shows without notification will also be charged in full. The Instructor reserves the right to cancel or reschedule sessions due to illness, emergency, or unforeseen circumstances, and will provide as much advance notice as possible.

Disclaimer & Liability Waiver

Not a Medical Service: Qigong instruction provided by House of Guanyin / Moving Qigong LLC is intended for wellness, mindfulness, and general health improvement purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment. Coach Katrina is not a licensed medical professional.

Medical Clearance: The Client is strongly advised to consult with a qualified healthcare provider before beginning any new physical or movement-based practice, particularly if the Client has pre-existing medical conditions, injuries, or physical limitations.

Assumption of Risk: The Client acknowledges that participation in Qigong sessions involves physical movement and voluntarily assumes all risks associated with such participation. The Client agrees to inform the Instructor of any physical conditions, injuries, or limitations prior to sessions.

Limitation of Liability: Moving Qigong LLC, dba House of Guanyin, and Instructor Katrina T. Olurinde shall not be held liable for any injury, illness, loss, or damage of any kind arising from participation in Qigong sessions, whether virtual or in-person.

Results Not Guaranteed: Individual results from Qigong practice vary. No specific health outcomes or results are guaranteed by the Instructor or Moving Qigong LLC.

Client Confidentiality & Privacy Agreement

This Confidentiality and Privacy Agreement ("Agreement") is entered into between Moving Qigong LLC dba House of Guanyin, represented by Instructor Katrina T. Olurinde (Coach Katrina), and the Client identified below. This Agreement governs the collection, use, storage, and protection of all personal and health-related information shared during the course of the Client's engagement with House of Guanyin.

Terms of Confidentiality

1. Protection of Personal Information. All personal information provided by the Client, including but not limited to full name, contact details, date of birth, gender, and health information, will be kept strictly confidential by Moving Qigong LLC and Coach Katrina. This information will not be shared, sold, disclosed, or distributed to any third party without the express written consent of the Client, except as required by law.

2. Health & Medical Information. Any physical, medical, or health-related information disclosed by the Client — whether verbally, in writing, or through the intake form — is considered protected health information. This information is used solely to customize and ensure the safety of Qigong sessions and will not be disclosed to any outside party without the Client's explicit written permission.

3. Session Privacy. The content, discussions, personal disclosures, and activities that occur during Qigong sessions — whether virtual or in-person — are considered private and confidential. Coach Katrina will not share, record (without consent), or disclose session content to any third party.

4. Electronic Communications. All invoices, session confirmations, and correspondence are conducted electronically via email. The Client's email address and contact information will be used solely for business communications related to House of Guanyin services and will not be shared with or sold to third parties for marketing or any other purposes.

5. Data Storage & Security. Client records, intake forms, agreements, and related documents are stored securely by Moving Qigong LLC. Reasonable measures are taken to protect client data from unauthorized access, loss, or misuse. Physical documents, if applicable, are stored in a secure, locked location.

6. Photography, Video & Testimonials. No photographs, video recordings, or audio recordings of the Client will be taken during sessions without the Client's prior written consent. The Client's name, likeness, or testimonials will not be used for promotional or marketing purposes without explicit written authorization from the Client.

7. Exceptions to Confidentiality. Confidentiality may be limited in the following circumstances: (a) if the Client discloses information that suggests imminent risk of harm to themselves or others; (b) if disclosure is required by applicable law or legal process; or (c) if the Client provides written authorization for specific disclosures. In all such cases, only the minimum necessary information will be disclosed.

8. Client Rights. The Client has the right to request access to their personal information held by Moving Qigong LLC, to request corrections to inaccurate information, and to request deletion of their records upon termination of services, subject to any legal retention requirements.

9. Instructor's Obligations. Coach Katrina agrees to uphold the highest standards of professionalism and discretion in handling all client information. This obligation of confidentiality continues indefinitely, even after the conclusion of the Client's services with House of Guanyin.

10. Mutual Respect. The Client also agrees to maintain discretion regarding any proprietary instructional content, techniques, or methodologies shared by Coach Katrina during sessions. This includes refraining from sharing, reproducing, or distributing session materials without prior written consent from Moving Qigong LLC.

11. Duration of Agreement. This Agreement is effective upon signing and remains in force for the entire duration of the Client's engagement with House of Guanyin and indefinitely thereafter with respect to information already collected.

12. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of Alabama. Any disputes arising under this Agreement shall be resolved in the appropriate courts of Alabama.

📋 Client Acknowledgment & Consent

By signing below, the Client acknowledges that they have read and fully understand this Confidentiality and Privacy Agreement. The Client consents to the collection and use of their personal and health information as described above, for the sole purpose of facilitating Qigong instruction services provided by Moving Qigong LLC dba House of Guanyin.

The Client understands they may withdraw this consent at any time in writing, which may affect the ability of House of Guanyin to continue providing services.
